• (In office or telemedicine) 45 Minute Follow Up Visit - $375.00
• (In office or telemedicine) 60 Minute Follow Up Visit - $500.00
• EMG Study - $500.00

We can make special arrangements for home visit to either an assisted living home or skilled nursing facility. Call for any inquiries regarding home visits.
